The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death. 82 South, Inc. has put in place preventative measures to reduce the spread of COVID-19; however, 82 South, Inc. cannot guarantee that you and your guests will not become infected with COVID-19 while attending the event- Tricks & Screams Produced by 82 South Inc. Therefore, if you and your guests choose to attend the event and/or enter onto rented premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

I agree to a temperature check upon arrival. If my temperature is over 100.4 degrees, I understand that I could be denied access to the event.

I agree to self-assess and monitor my health two weeks up to the event and on event day.

I agree to immediately notify 82 South, Inc. staff should I begin to exhibit any COVID19 symptoms including: fever, chills, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea.

I understand that I will be signing a release waiver upon entry into the event.

ASSUMPTION OF RISK:

I have read and understood the above warning concerning COVID-19. I hereby voluntarily choose to accept all foregoing risks of contracting COVID-19 for myself in order to attend Tricks & Screams. I accept sole responsibility for any injury to my myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my participation in events (“Claims”).

WAIVER OF LAWSUIT/LIABILITY:

I hereby forever release and waive my right to bring suit against 82 South, Inc. and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing attending Tricks & Screams. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE
